Important information on General practice & your GP
Tips to get the most of your appointment
To all our patients we provide medical services led by the GPs and the integrated Primary Care Nurse Team. We offer as much extended care as is appropriate and feasible. Examples of extended services provided from the Practice are:
Regular clinics include CHD, hypertension, diabetes, COPD, asthma, spirometry, antenatal care, child health, smokestop and stroke/TIA.
Minor surgery is undertaken. We are keen to improve and add to the services we provide for our patients. We are a dispensing Practice for around 95% of our patients and operate a repeat prescription drop off/pick up system in several villages.
Patients who suffer from Diabetes, Heart Disease, Hypertension, Chronic Kidney Disease or any other chronic disease are reviewed regularly by our team of trained nurses.
If you are going abroad, please complete this form 8 weeks before you travel.
It's important to make this initial appointent as early as possible, at least six weeks before you travel, as a second appointment with the Travel Nurse will be required for you to receive the vaccinations, and your second appointment needs to be at least two weeks before you travel to all ow the vaccines to work.
Vaccinations for Hepatitis A, Tetanus/Diphtheria/Polio and Typhoid can be prescribed and so are free of charge, but for those vaccinations we cannot prescribe, private charges are made.
There is further information about countries and vaccinations required on the links below:
To help us offer the appropriate advice, please fill out the online form before coming to see the nurse.
Travelling in Europe
If you are travelling to Europe a very useful booklet has been published with advice and guidance to help you get the most out of your holiday. To visit please click:- http://ec.europa.eu/publications/booklets/eu_glance/86/en.pdf (this is a large document and may take a minute or two to view)
Some services provided in the practice are not funded by the NHS and in these cases a private fee is charged as follows. Charges made are in line with BMA recommendations. VAT is included where applicable
This applies to copies of all items from your medical records
Rabies (course of three)
There are occasions when patients need to be assessed by a doctor which might involve intimate examinations.
This Practice is committed to putting patients at ease whenever possible, and if you wish a chaperone to be present during your examination please do not hesitate to ask the doctor.
It may not be possible for such a chaperone to be provided immediately and you might have to return for the examination to be carried out at a mutually convenient time.
Trust is important in the relationship between GP and patient and we would, at all times, wish you to feel able to ask for a chaperone, should you require it.
Enter all or part of your postcode in the box below and click one of the buttons
